Deliver PT care with home visits as needed.
Educate participants and caregivers on exercises and DME.
Collaborate with the Interdisciplinary Team for care planning.
Support DME inventory and infection control.
Foster a center culture of belonging and safety.

🎯 Requirements

Doctorate of Physical Therapy (DPT) required.
Bachelor’s Degree in Physical Therapy.
Active California PT license.
Minimum 1 year experience with frail/elderly.
Ability to travel between work sites.
Bilingual Spanish/English preferred.
