Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, ambulation, strength.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and revise individualized care plans, incl. manual therapy, gait/balance training.
• Document observations and interventions using EMR on a tablet.
• Educate clients and families on rehabilitative care and safety for independent living.

🎯 Requirements

• Current AZ Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
