📋 Description

• Make home visits to clients in designated territories.
• Conduct diagnostic tests and measurements (mobility, strength, balance).
• Implement wound care with evidence-based dressings.
• Develop and implement individualized PT care plans.
• Continually assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families; document observations and interventions in EMR.

🎯 Requirements

• AZ Physical Therapist (PT) license.
• Minimum 1 year of PT clinical experience.
• Graduate from APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
