Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, ROM, balance, strength, and safety.
• Wound care with dressings and topical agents.
• Develop and implement individualized PT care plans, including gait and balance training.
• Assess and revise care plans; coordinate with nursing and the care team.
• Educate clients and families on rehabilitative care for safety and independence.

🎯 Requirements

• Current AZ PT license.
• Minimum one year PT experience.
• Graduate from an APTA/CAAHEA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
