📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care with evidence-based dressings.
• Develop and adjust care plans: manual therapy, gait training.
• Monitor progress; revise care plans with nursing.
• Educate clients and families for safe, independent living.

🎯 Requirements

• A current SC Physical Therapist - PT license.
• Minimum of one year PT experience.
• Graduation from an APTA- or AMA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
