Physical Therapist

📋 Description

• Provide skilled physical therapy in patients' homes.
• Complete PT evaluations (ROM, strength, balance, mobility).
• Develop individualized treatment plans to maximize function.
• Deliver therapeutic exercise and gait/balance training.
• Document progress in EMR and coordinate with the care team.
• Educate patients and caregivers on home exercise and mobility safety.

🎯 Requirements

• Current NJ PT license.
• Minimum 1 year of PT experience.
• Graduate of an accredited PT program (APTA or equivalent).
• EMR experience comfortable.
• Strong assessment, communication, organizational, and critical thinking.
• Willingness to travel across Monmouth, Somerset, Union Counties.
