Physical Therapist

📋 Description

• Conduct PT assessments at facility and home visits to define goals
• Participate in IDT meetings to develop care plans
• Deliver and document interventions per care plans
• Provide active and passive exercise, gait, and ADL training
• Supervise Therapy Assistants and implement FMP
• Train staff, participants, and caregivers on equipment

🎯 Requirements

• Bachelor's degree in physical therapy; master's or doctoral preferred
• 2 years in clinical setting with frail/elderly population preferred
• Valid California PT license or license eligibility
• CPR and First Aid certification within 180 days of hire
