Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, ROM, transfer status, balance, safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy and gait training.
• Continuously assess and revise PT care plan with nursing and the multidisciplinary team.
• Educate clients and families and document observations in EMR on a tablet.

🎯 Requirements

• Current SC Physical Therapist license.
• Minimum of 1 year of recent PT experience.
• Graduate from an APTA-approved program or AMA CAAHEA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
