Physical Therapist

📋 Description

• Conduct PT assessments in facilities and at times in participants' homes.
• Actively participate in IDT meetings to develop care plans.
• Deliver and document interventions per care plans.
• Provide active and passive exercises, gait and ADL training.
• Supervise Therapy Assistants and guide FMP implementation.
• Train staff, participants and families on equipment use.

🎯 Requirements

• Bachelor’s degree in physical therapy; advanced degree preferred.
• Two years in clinical setting with frail or elderly population preferred.
• Valid California Physical Therapy License or license eligibility.
• Must obtain CPR and First Aid within 180 days; maintain certification thereafter.
• CPR and First Aid certification upon hire preferred.
