Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/ROM, ambulation, strength, balance, safety.
• Develop and implement individualized care plans, including manual therapy, gait training, balance.
• Continuously assess and revise PT care plans with nursing and the team.
• Educate clients and families on rehabilitative care for safety and independence.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• A current VA Physical Therapist - PT license.
• Graduation from an APTA-approved program or AMA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
