Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Implement wound care with evidence-based dressings and topical agents.
• Develop individualized care plans, including exercises, gait training, and balance.
• Continuously assess and revise the PT care plan with the care team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• A current Pennsylvania Physical Therapist (PT) license.
• Minimum one year of PT experience.
• Graduate from an APTA/CAAHEA/AMA-accredited program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
