Physical Therapist

📋 Description

• Conduct PT assessments in facility and in-home to define care needs and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per participant care plans.
• Provide active and passive exercise, gait training, ADL therapy, and modalities.
• Supervise Therapy Assistants and implement Functional Maintenance Programs (FMP).
• Provide training to staff, participants, and caregivers on therapeutic equipment.

🎯 Requirements

• Bachelor’s degree in physical therapy; master's/doctoral preferred.
• Valid California Physical Therapy License or license eligibility.
• CPR and First Aid certification within 180 days of hire.
• Experience in outpatient, acute, inpatient, or skilled nursing settings preferred.
