📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and mobility/strength assessments.
• Implement wound care and protective interventions.
• Develop and adjust individualized care plans and therapies.
• Reassess care plans and coordinate with nursing.
• Educate clients and families on rehabilitation and safety.

🎯 Requirements

• VA PT license (PT).
• Graduate from APTA-approved PT program or AMA/COA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Experience in home health or geriatric care preferred.
