Physical Therapist

📋 Description

• Conduct PT assessments in facility and home to define care needs and goals
• Participate in IDT meetings to develop care plans
• Deliver and document interventions per care plans
• Provide PT interventions including exercises, gait, ADLs, and modalities
• Supervise Therapists Assistants to implement Functional Maintenance Programs
• Train staff and participants on therapeutic equipment use

🎯 Requirements

• Bachelor's degree in physical therapy; master's or doctoral preferred
• Valid CA PT license or license eligibility
• CPR and First Aid within 180 days of hire; certification maintained thereafter
