Physical Therapist

📋 Description

• Provide home health Physical Therapy in patients' homes in Bucks County, PA
• Assess mobility, strength, balance, endurance, and safety; develop individualized care plans
• Perform diagnostic tests and measurements (ROM, gait, transfers)
• Implement wound care using evidence-based dressings and topical agents
• Educate patients and families to promote health and independent living
• Document observations and interventions using a tablet and collaborate with nursing

🎯 Requirements

• Active PA PT license
• Previous home health experience welcomes; training provided
• Ability to travel to cases; compassionate, attentive communicator
• Deliver care with compassion, excellence, and reliability
