Make home visits to clients in designated territories.
Perform diagnostic tests and measurements (mobility, strength, balance).
Implement wound care with evidence-based dressings.
Develop and revise individualized care plans (manual therapy, gait, balance).
Continually assess and revise the care plan with nursing in the multidisciplinary team.
Educate clients and families in rehabilitative care to promote health, safety and independence.

🎯 Requirements

Current Virginia PT license.
Graduate from an APTA-approved PT program.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
