Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including ROM, transfer status, gait, strength, balance
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized care plans with manual therapy, gait training and balance.
• Continuously assess and revise the PT care plan; coordinate with nursing in a multidisciplinary
• Educate clients and family on rehabilitative care to promote independence.

🎯 Requirements

• Current NH Physical Therapist license.
• Minimum one year PT experience.
• Graduate from an APTA/CAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
