📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer status, balance, gait, strength
• Implement wound care and protective interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait training, balance, and
• Continuously assess and revise the care plan; collaborate with nursing on the multidisciplinary
• Educate clients and family members to promote health, safety, and independent living.

🎯 Requirements

• MA Physical Therapist license (current).
• Minimum of one year PT experience.
• Graduate of an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
