📋 Description

Provide home health visits in designated Warrenton area territories
Assess patient mobility, gait, balance, strength, and endurance
Develop and implement individualized care plans and interventions
Educate clients and families on rehabilitative care for independent living
Document observations and interventions using EMR
Collaborate with nursing and multidisciplinary team

🎯 Requirements

Active VA Physical Therapist license
Minimum of 1 year PT experience
Ability to work independently; strong organization and problem solving
Good interpersonal skills; comfortable with EMR systems
Strong manual therapy, gait, balance, and safety assessment skills
Graduation from an APTA/CAFHEA accredited program
