📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, ROM, transfer, balance, and safety assessments.
• Implement wound care using evidence-based dressings.
• Develop and revise care plans; include gait, balance, and manual therapy.
• Continuously assess and revise care plan with nursing in MDT.
• Educate clients and families for health and independence.

🎯 Requirements

• Current MA PT license.
• Minimum 1 year PT experience.
• Graduate from an APTA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
