📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility, strength, balance, and safety assessments.
• Provide wound care using evidence-based dressings and topical agents.
• Develop individualized plans with manual therapy, gait and balance training.
• Assess and revise care plans; collaborate with nursing.
• Educate clients and families on rehabilitation for independence.

🎯 Requirements

• One year of licensed Physical Therapist experience.
• Active Pennsylvania PT license.
• Graduation from an APTA-approved program per transcript/diploma.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
