📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, balance, strength, and safety assessments.
• Provide wound care with evidence-based dressings.
• Develop individualized care plans: manual therapy, gait, balance.
• Revise care plans; collaborate with nursing.
• Educate clients and families for safe, independent living.
• Document observations and interventions on a tablet.

🎯 Requirements

• Maryland Physical Therapist (PT) license.
• Graduation from an APTA/CAHEA accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
