Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, transfer status, stability, balance, strength, endurance, safety).
• Implement wound prevention and repair with dressings and topical agents.
• Develop individualized care plans, including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health, safety, and independent living.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum 1 year PT experience.
• Degree from APTA/CAAHEA-accredited program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
