Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, gait, balance, and safety assessments.
• Implement wound care and protective interventions.
• Develop and revise individualized PT care plans.
• Educate clients and families on rehab and safety.
• Document observations/interventions in an EMR tablet.

🎯 Requirements

• A current STATE Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program or AMA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
