Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Do diagnostic tests: mobility, ROM, transfers, balance, strength, safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait, balance.
• Assess and revise PT care plan with nursing in the multidisciplinary team.
• Educate clients and families to promote health, safety, independence.
• Document observations and interventions using a tablet EMR.

🎯 Requirements

• Indiana PT license.
• Minimum 1 year PT experience.
• Graduate from APTA-approved program or AMA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
