📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, range of motion, strength and balance assessments.
• Implement wound care and preventive interventions.
• Develop and update individualized PT care plans.
• Educate clients and families for independent living.
• Document observations using EMR tablet.

🎯 Requirements

• PA Physical Therapist license.
• Graduation from CAPTE-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal and problem solving skills.
• Solid computer skills; EMR experience preferred.
