Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measures (mobility, ROM, transfer status, strength, balance).
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized care plans with manual therapy, gait and balance training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and family on rehabilitative care for health, safety, independence.

🎯 Requirements

• A PA PT license in good standing.
• Minimum of one year of recent PT experience.
• Graduation from an accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
