Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, e.g., mobility and ROM, transfer status, balance
• Implement wound care interventions and use evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait, balance training, and
• Continually assess and revise the PT care plan and collaborate with nursing in multidisciplinary
• Educate clients and family on rehabilitative care and independent living.

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from APTA-approved program or AMA-accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
