📋 Description

  • Provide PT eval and treatment for PACE participants across care levels.
  • Develop individualized treatment plans and conduct home visits as needed.
  • Educate participants and caregivers on exercise programs and DME use.
  • Serve on Interdisciplinary Team (IDT) and coordinate care with documentation.
  • Support PT program standardization, team education, and quality audits.
  • Maintain documentation meeting regulatory requirements and infection control.

🎯 Requirements

  • Bachelor’s Degree in Physical Therapy.
  • Active California PT license.
  • Completion of an APTA-accredited PT program or CMS-approved equivalent.
  • Minimum 1 year experience with frail/elderly populations.
  • Bilingual Spanish/English strongly preferred; other languages a plus.
  • Ability to travel between work sites; strong documentation.
