Physical Therapist - Per Diem, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (ROM, gait, balance, strength).
• Implement wound care and dressings per evidence-based protocols.
• Develop and revise care plans (manual therapy, gait, balance).
• Educate clients and families on rehab, safety, and independence.
• Document observations and interventions in the EMR using a touch screen.

🎯 Requirements

• Current MD PT license.
• Graduate from an APTA-approved PT program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
