Physical Therapist (PT), Full Time, Home Health

📋 Description

• Visit a variety of clients in their own homes to assist recovery from injury, illness, surgery, or
• Provide 1:1 direct care in client homes with flexible full-time, part-time, or per diem options.
• Perform diagnostic tests and measurements (mobility, range of joints, transfer status, gait
• Develop and implement individualized care plans with manual therapy, gait training, balance, and
• Document observations and evaluations using a state-of-the-art touchpad tablet; revise care plans
• Educate clients and family members on rehabilitative care to promote health and independence.

🎯 Requirements

• Active and current PT license
• One year of clinical experience preferred
• Home health care experience welcomed but not required; training provided
• Ability to travel locally to cases as assigned
