Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, gait, and safety assessments.
• Implement wound care and dressings to promote healing.
• Develop and update individualized care plans with manual therapy and gait training.
• Assess and revise care plans; collaborate with nursing in multidisciplinary team.
• Educate clients and families on rehabilitative care for independence.

🎯 Requirements

• Current North Carolina PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA/AMA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
