Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility and ROM, strength, balance, and safety assessments.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and adjust individualized care plans including manual therapy and gait training.
• Continuously assess and revise the PT plan with nursing in the care team.
• Educate clients and families to promote health, safety, and independent living.

🎯 Requirements

• North Carolina Physical Therapist license.
• Graduation from an APTA- or AMA-accredited PT program (transcript or diploma required).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Experience with electronic medical records (EMR) preferred.
