Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, ROM, strength, balance.
• Implement wound care with dressings and topical agents.
• Develop and implement individualized PT care plans.
• Assess, revise care plans, and document observations on a tablet.
• Educate clients and families on rehab and safety for independence.

🎯 Requirements

• Current North Carolina PT license.
• Graduate from an APTA-approved program or AMA CAAHEA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
