📋 Description

• Make home visits to clients in designated territories.
• Perform mobility tests and measurements (ROM, strength, balance, safety).
• Implement wound care with evidence-based dressings and topical agents.
• Develop individualized care plans with manual therapy, gait, balance training.
• Collaborate with nursing to reassess the care plan.
• Document care in EMR using a touchpad tablet.

🎯 Requirements

• North Carolina Physical Therapist license.
• Graduation from APTA-approved program or AMA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Strong computer skills with EMR experience preferred.
