Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform ROM, mobility, transfer status, balance, safety assessments.
• Implement wound care and dressings per evidence-based protocols.
• Develop and implement care plans, including manual therapy and gait/balance training.
• Continuously assess and revise PT care plans with the multidisciplinary team.
• Educate clients and families in rehabilitative care for health and independence.
• Accurately document observations and interventions using a touchpad tablet.

🎯 Requirements

• Current PA Physical Therapist - PT license.
• Minimum one year of recent PT experience.
• Graduate from an APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem solving skills.
• Solid computer skills; EMR experience preferred.
