Physical Therapist, (PT) - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, safety).
• Implement wound care with evidence-based dressings and topical agents.
• Develop and implement care plans (manual therapy, gait training, balance).
• Assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families on rehabilitation and safety.

🎯 Requirements

• A current North Carolina Physical Therapist license.
• Graduate from a CAPTE-approved PT program (APTA/AMA CAAHEA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
