Physical Therapist, PT, Home Health Visits

📋 Description

• Provide home visits to clients in designated geographic territories (Northeast Philadelphia).
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, ambulation, strength
• Implement wound care and protective interventions to promote healing using evidence-based dressings
• Develop and execute individualized care plans with manual therapy, gait training, balance and other
• Continuously assess and revise PT care plans; collaborate with nursing on multidisciplinary care
• Educate clients and caregivers on rehabilitative care to promote health and independent living.

🎯 Requirements

• PA Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from APTA-accredited program or equivalent (as indicated by transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
