Physical Therapist, (PT) - Home Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility ROM, gait, strength, balance, safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait, balance.
• Continuously assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients, families, or representatives in rehabilitative care for independence.

🎯 Requirements

• A current North Carolina PT license.
• A minimum of one year of recent work experience as a PT.
• Graduation from an APTA- or CAAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.
