Physical Therapist, PT Home Visits - Weekend

📋 Description

• Provide home health PT care during weekend Start of Care admissions and ongoing visits.
• Perform mobility, strength, balance, and safety assessments at patient homes.
• Develop individualized, evidence-based treatment plans and execute gait, balance, and therapeutic
• Document care using EMR system; collaborate with a supportive local team.
• Maintain autonomy with work‑life balance in a per diem weekend role.

🎯 Requirements

• Current Colorado PT license (or eligibility).
• Graduation from an accredited PT program.
• Experience with Medicare Certified Home Health and OASIS preferred.
• Comfort with EMR systems; strong documentation skills.
• Weekend SOC visits experience or willingness to start with weekend schedule.
