  • Make home visits to clients in designated geographic territories.
  • Perform diagnostics: mobility, ROM, transfers, ambulation, strength, balance, safety.
  • Implement wound care with evidence-based dressings and topical agents.
  • Develop and implement individualized PT plans, including manual therapy, gait and balance training.
  • Continuously assess and revise PT care plan with nursing and document care.
  • Educate clients and families on rehab to promote independence.

🎯 Requirements

  • North Carolina Physical Therapist (PT) license.
  • Minimum 1 year of recent PT experience.
  • Graduate of an APTA-approved or AMA-accredited program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
