Physical Therapist, PT, Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfers, balance, gait, strength).
• Implement wound care with evidence-based dressings and topical agents.
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Continuously assess and revise PT care plans with nursing in the multidisciplinary care plan.
• Educate clients and families on rehabilitative care to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations in EMR.

🎯 Requirements

• Current DE PT license.
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
