Physical Therapist, PT, Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, ambulation, strength, balance, safety.
• Implement wound care and protective interventions using dressings and topical agents.
• Develop and implement individualized care plans: manual therapy, gait training, balance.
• Assess and revise PT care plan; collaborate with nursing on multidisciplinary plan.

🎯 Requirements

• Current OH Physical Therapist license.
• Minimum of one year PT experience.
• Graduate of APTA-approved program or equivalent.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
