Physical Therapist- PT Visits

📋 Description

• Provide PT services to clients in the home and create treatment plans
• Perform mobility, ROM, balance and safety assessments
• Implement individualized care plans with exercises and gait training
• Educate clients and families to promote health and independence
• Document observations and interventions using an EMR tablet
• Collaborate with a multidisciplinary team for home health care

🎯 Requirements

• A current Physical Therapist license in Hawaii
• Graduation from an APTA-approved PT program
• Solid computer skills; EMR experience preferred
• Ability to work in a home health setting
