Provide skilled PT evaluations and treatment during home/community visits.
Assess mobility, gait, balance, strength, ROM, and transfers.
Develop and implement individualized treatment plans with evidence-based interventions.
Perform skin integrity assessments and wound-prevention interventions.
Evaluate progress and modify plans for optimal outcomes.
Collaborate with nursing and interdisciplinary team.

🎯 Requirements

Current New Jersey PT license.
Graduate of an accredited PT program (APTA or equivalent).
Ability to work independently and manage your schedule.
Excellent communication, organizational, and problem-solving skills.
Experience with EMR systems preferred.
