Physical Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility tests, transfers, gait, balance, strength, and safety assessments.
• Implement wound care and dressings using evidence-based methods.
• Develop and implement individualized care plans including manual therapy, gait and balance training.
• Continually assess and revise care plans; coordinate with nursing.
• Educate clients and families in rehabilitative care and safety.

🎯 Requirements

• Maryland Physical Therapist license.
• Graduate of APTA-approved program (per transcripts).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem solving skills.
• Solid computer skills; EMR experience preferred.
