  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements for mobility, strength, balance, and safety.
  • Implement wound care and protective interventions with evidence-based dressings and agents.

  • Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.

🎯 Requirements

  • Pennsylvania Physical Therapist license.
  • Minimum of one year of clinical PT experience.
  • Graduation from an APTA/CAAHEA-approved program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
