📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status
• Implement integumentary protective interventions to prevent wounds and promote wound repair using
• Develop and implement individualized care plans including manual therapy, gait training, balance
• Continually assess and revise the PT care plan and collaborate with nursing in the

🎯 Requirements

• Current VA Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or CMAA accredited program as indicated by
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
