Visit clients in their home to aid recovery from injury, illness, surgery, or hospitalization
Develop and implement individualized care plans with exercises, gait training, and balance work
Continually assess and revise the care plan in collaboration with nursing
Progress clients from discharge to goal completion at home
Provide education to patients and families

🎯 Requirements

Active and current PTA license
Graduate from a two-year APTA-approved program
Two years PTA experience and proficiency exam as applicable
At least one year of work under supervision of a licensed PTA
Home healthcare experience welcomed; training provided
Compassionate, attentive communication and reliable patient care
