Primary Care Nurse Practitioner

📋 Description

• Provide ongoing primary and preventive care to older adults
• Manage and grow a dedicated patient panel
• Deliver relationship-driven, patient-centered care in community settings
• Collaborate with interdisciplinary clinical and operational teams
• Support preventative care, chronic disease management, and care coordination
• Maintain accurate EHR documentation and coding specificity

🎯 Requirements

• Active, unrestricted Nurse Practitioner license
• Board certification (AANPCB or ANCC)
• Active DEA license and controlled substance registration
• 3+ years clinical NP experience, preferably with aging populations
• Basic Life Support (BLS) certification
• Valid driver's license and reliable transportation
